What does an Intensive Case Manager for the Reaching Home Division do?

The Case Managers will provide trauma informed, intensive case management services to women, families, and youth who are experiencing chronic homelessness.

  • Successful candidates will work with service users in the community to secure permanent housing and will provide ongoing case management supports for up to 24 months;
  • The case managers will work closely with partner agencies to leverage and coordinate the assets and services available in Hamilton to ensure that service users have their individualized housing and support needs met;
  • The successful candidates will have a demonstrated knowledge of Housing First frameworks and the ability to assist service users with system navigation;
  • Work takes place in the office, overflow hotels, emergency shelters, and in the community;
  • Will accompany service users to community appointments, apartment viewings, and will attend service users’ homes for monthly home visits;
  • Must be willing and able to assist service users in a hands-on capacity;
  • Time management skills are necessary in a high energy, changing environment;
  • Ability to work independently

Requirements & Prerequisites

What qualifications are required to be an Intensive Case Manager?

  • Minimum 2-years related experience or equivalent combination of education, training and experience;
  • Post-secondary education in social services or related field is required;
  • Fluency in a 2nd language is an asset;
  • Proficiency in Microsoft Office programs and strong written and oral communication skills required;
  • CPR certification or willingness to be certified is required;
  • Valid G license required;
  • An in depth understanding of trauma, addictions, mental health, and homelessness is required;
  • Creative problem solving and advocacy skills required.

 

**Shifts are Day shifts (9 a.m. to 5 p.m.) and one evening shift per week (11 a.m. – 7 p.m.) and may include some weekends. Some flexibility in work hours may be required.
