Good Shepherd Centres Hamilton, a ministry of the Hospitaller Order of St. John of God, strives to enhance the quality of life for all who seek assistance and support in our community. An opportunity currently exists in a new pilot program in the role of:
Case Manager
Temporary Full Time
The Case Manager attached to the Transitional Bed Program will work as a member of a multi-disciplinary team that provides health care and human services to individuals who require short term residential, health and case management services upon discharge from hospital. The case manager will assist individuals in accessing supports and resources in the community to support optimal health, well-being and community function. This position requires tenacity, creativity and a deep commitment to person-centred service.
Qualifications:
• Demonstrated interest and experience in providing case management services to vulnerable adults impacted by poverty, homelessness, addiction, mental illness, and/or trauma
• Familiarity with Hamilton’s homeless serving system (including shelters, coordinated access, By-Name-Priority List, and evidence-based housing interventions e.g. Housing First)
• Significant experience in supporting individuals living with multiple, complex challenges impacting their health, safety and stability.
• Demonstrated commitment to trauma-informed care, harm reduction and working within an integrated anti-racism/anti-oppression framework
• Diploma or degree in social service/social work
• Fluency in a 2nd language an asset
• CPR certification or willingness to be certified is required. Must have a valid G driver’s licence.
• Availability to work flexible hours including some weekend work.
