Job Description

 Good Shepherd Centres Hamilton, a ministry of the Hospitaller Order of St. John of God, strives to enhance the quality of life for all who seek assistance and support in our community.  Two temporary full-time position, with the Intensive Case Management Program, at the Family Centre currently exists in the role of:

Temporary Full Time Intensive Case Manager

The Case Manager will provide trauma informed, intensive case management services to families who are chronically and episodically homeless from a Housing First Model. They will work with families in the community to achieve housing stability and provide case management supports. As part of a collaborative program, the case manager will work closely with partner agencies to leverage and coordinate the assets and services available in Hamilton to ensure that families have their individualized housing and support needs met.   The successful candidate will have a demonstrated knowledge of Housing First frameworks and various governmental and non-governmental systems as well as an understanding of how to assist families in successful systems navigation. An in depth understanding of trauma as it relates to family homelessness is also necessary.

Applicants must have the ability to work within a person centered, anti-racist / anti-oppressive framework and have a minimum of 2 years’ experience with housing work or in an emergency residential setting. Experience working with clients in the community is necessary. Applicants must possess a related post-secondary education, valid C.P.R. certification and have a demonstrated commitment to the mission values of availability, responsibility, adaptability, quality and dignity. Possession of a valid G class Ontario driver’s license is required and ability to drive the company vehicles. The successful candidate will have a flexible schedule. Fluency in French and/or a language in addition to English is an asset.
