Health Navigator Responsibilities:
- Assess, plan and evaluate individualized care for residents regularly
- Coordinate resident assessments and service plans
- Liaise with healthcare professionals and maintain accurate records
- Respond to health emergencies and ensure compliance with safety policies
- Coach Care Services team members
Days & Hours of Work: Monday – Friday, Day shift
Compensation: $60-65K annually
Benefits: Health and Dental, 3 weeks’ vacation, sick and float days
Our location is Bus Accessible (with a 20 minute walk from the bus stop)
Requirements & Prerequisites
- RN or RPN designation with current registration
- Covid-19 Vaccination is mandatory
- Valid First Aid/CPR certification
- Vulnerable Sector Record Check
- TB Test
- Knowledge of seniors’ retirement housing and relevant legislation
- Strong communication and organizational skills
