Job Description
This is a permanent part-time role, offering 15 hours per week (30 hours bi-weekly). You will be required to work 2 weekdays and every other weekend (4 shifts bi-weekly). Shifts are 8 hours long. Two shift options are available (on a first come first served basis): Day shift: 7am-3pm / Evening shift: 3pm-11pm. Compensation starts at $29 per hour.

The Registered Practical Nurse reports to the Director of Care and is expected to:
- Coordinate and oversee all aspects of resident-focused delivery of care.
- Provide direct supervision, leadership, and education to other members of the healthcare team.
- Act as a resource advocate for residents, families, and staff.
- Promote the values, goals, and philosophy of the Home.
Responsibilities:
- Develop and modify plans of care in response to residents’ individualized health care needs and changes in health care status.
- Liaise with other healthcare personnel, agencies, and families.
- Maintain good relationships with Medical/Support/Home staff, residents, families, and the general public.
- Supervise all activities on the unit and take charge as required.
- Monitor performance and complete performance evaluations for the PSWs and RPNs.
- Direct, assign, and evaluate the work of the PSWs.
- May be in charge of the Home as required, utilizing leadership skills to identify and solve problems on the unit.
- Ensure care delivered is consistent with the resident Bill of Rights.
- Understand the nature and meaning of quality indicators used by the Home and which indicators are publicly reported.
- Participate in the improvement of the indicators and achieve satisfactory results.
- Order and ensure nursing supplies and equipment are properly and efficiently used.
- Actively listen to residents and family concerns, taking appropriate actions to resolve issues in accordance with Home policies.
- Conduct unit rounds at the beginning and end of shifts.
- Process Physician orders and administer medications and treatments in compliance with the policies of the Pharmacy provider, the Home, and the Standards of the College of Nurses. Document and report all errors and omissions.
- Maintain accurate and complete records of nursing observations and care in compliance with legislative requirements and the policies of the Home.
- Assess changes in health status and report changes to Substitute Decision Maker (SDM) or POA, and the interdisciplinary team.
- Complete health care assessments (RAI-MDS and other assessments), develop and update care plans for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
- Demonstrate understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home.
- Perform other duties as assigned and/or indicated in the Job Task Inventory.
Requirements & Prerequisites
Qualifications:
- Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse, or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction other than Ontario, holding in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as per Bill 60 exemption.
- Basic CPR.
- Experience in a long-term care setting is an asset.
- Ability to work with and relate to interdisciplinary team members.
- Well-developed written and oral communication skills.
- Knowledge of RAI-MDS – AIS certified, or must complete certification successfully within one (1) year of hire.
- Evidence of focusing on people’s feelings when interacting and turning tasks into positive social interactions.
- Undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
- Provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
- Submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements.
- Provide two supervisory references.
- COVID-19 vaccination is a condition of employment.
